CHC is seeking a PRN Family Nurse Practitioner/Physician Assistant Health for an on-site health center located in Northlake, IL. This health center provides Occupational Health, Urgent Care, Primary Care and Health Promotion services to employees of a local employer.
Responsibilities of the Family Nurse Practitioner/Physician Assistant may include but are not limited to:
Wellness coaching and advising
Ability to collaborate with the PCP around wellness outcomes for high risk members to create personal health plan, and drive positive behavior changes
Participate in marketing activities tied to wellness, and partner with the benefits team to create awareness around wellness
